Kindergarten Summer Days Download Kindergarten Camp Registration Form 2008 Registrations are now being accepted in the office. For Boys & Girls Ages 5-6 Years Nine One-Week Sessions June 23 - August 22 8:00am - 3:30pm Extended Care Available Fidelity House is offering a Kindergarten Summer Days for children who are entering or just completing Kindergarten.  There will be nine one-week sessions (Monday through Friday).  June 23 - 27  July 28 - August 1  June 30- July 3*  August 4 - 8  July 7 - 11  August 11- 15  July 14-18  August 18 - 22 July 21-25   (* no camp Friday, July 4). Hours will be 8:00 am - 3:30pm Extended Care is available until 5:30pm for an additional fee. Camp Site/Activities Activities will take place at Fidelity House’s additional Day Care building / play area (FiHo 2), Fidelity House and Minuteman Regional Technical High School in Lexington.  We also offer field trips and special events.  The camp offers a variety of recreational activities geared to the kindergarten-aged child including:  arts & crafts science discovery creative movement sport-indoor/outdoor  drama library visits local parks story time gymnastics cooking exploration/field trips music  contests & games swimming lessons  (available June 30-- Aug. 1 only) computer time Camp Registration Download Kindergarten Camp Registration Form At Initial Registration Please fill out the one page registration form per child.  A $50.00 NON-REFUNDABLE deposit per week (per child) is required to hold the spot.   It will be credited toward the camp fee and is not an additional fee. Registration deadline for each week of camp is the preceding Friday at 12:00 noon.  A $10 late fee will be applied to registrations after 12:00 noon.   There is limited enrollment for each session.   Enrollment is accepted on a first come, first serve basis. Before Attendance at K Summer Days Any BALANCE DUE for the upcoming week must be paid the preceding Friday. A DOCTOR signed/MEDICAL (Immunization) RECORD and Early Education and Care FORMS are mandatory & requested 2 WEEKS before your child attends camp.   All MEDICATIONS (short term/long term) require a medication authorization on file.  Please request the form if applicable.  Camp Fees One Week                       $180.00 Five (or more) Weeks       $170.00* *Per Week at Time of Initial Registration  Extended Care The PREPAID fees are listed below.  Please note your needs on your child’s registration form. 3:30 - 4:30        $25.00** 3:30 - 5:30        $50.00** Extended care for campers not previously signed up will be provided at a rate of $2.50 per 15 minutes, payable at pick-up time. Daily Checklist Campers should bring the following items to camp every day, regardless of the weather.  Also, all items should be clearly marked with the child's name. Lunch, Drinks, Snacks Sneakers, Bathing Suit Towel and Sunscreen Important Information Children should arrive at Fidelity House's Kindergarten day care building between 8:00A.M. and 8:20 A.M. daily. Please call Fidelity House by 8:25 A.M. any day your child will not be attending camp. If for any reason you do not want your child involved in the swimming program for the day or for any period of time, please notify us.  ALL MEDICATIONS (short term/long term) require a medication authorization on file. Please request the form if applicable.   Full Day Programs Kindergarten age children are welcome to participate in our Full Day Programs, offered on all days that school is not in session.  Summer Full Day Programs are offered before and after our Kindergarten Summer Days right up to the start of school in September.  A separate registration is required for those days.
